To allow others to pick up your reserved items, please read the service agreement at the bottom of the form, then enter the names of those you wish to designate in the "Designated Name" fields.

If you do not have an e-mail address then you cannot submit this online form. Instead please click here and print the form. Fill out the form and take it to your local library.
